Microsoft Forms is a simple online tool for creating surveys, questionnaires and quizzes.
Whilst we still recommend that staff and researchers use the more comprehensive Jisc Surveys tool for surveys that are more complex or collect sensitive or research data, Microsoft Forms offers a simple alternative for quick surveys and quizzes.
Microsoft Forms is available to all University staff and students.
You can access your Microsoft Forms via the Office 365 portal at https://office365.leeds.ac.uk
If your Form has a requirement for multiple people to access the data, we recommend that you use a Team or SharePoint site so that the Form is not owned by an individual account. For more information on creating and using Forms in Teams, please see the following Microsoft guidance.
