What is it?

Microsoft Forms is a simple online tool for creating surveys, questionnaires and quizzes.

Whilst we still recommend that staff and researchers use the more comprehensive Jisc Surveys tool for surveys that are more complex or collect sensitive or research data, Microsoft Forms offers a simple alternative for quick surveys and quizzes.

What do I use it for?

Microsoft Forms can be used within both learning & teaching and administration. Examples include:

Quizzes which show respondents the correct choices once they submit their answers
Simple surveys and questionnaires
Feedback forms
Registrations and events signup sheets

You can share surveys and quizzes created in Forms with anyone, including people outside of the University. Forms also integrates with Excel and OneDrive so that you can see results appear in an Excel spreadsheet in real time.

Please note, Microsoft Forms integration with PowerPoint is currently unavailable. If you wish to share Forms in a PowerPoint presentation, we recommend that you add the link to survey or quiz for students to access in their web browser.

Who is it available to?

Microsoft Forms is available to all University staff and students.

How do I access it?

You can access your Microsoft Forms via the Office 365 portal at https://office365.leeds.ac.uk

If your Form has a requirement for multiple people to access the data, we recommend that you use a Team or SharePoint site so that the Form is not owned by an individual account. For more information on creating and using Forms in Teams, please see the following Microsoft guidance.

Training

Microsoft Forms training is available via LinkedIn Learning and is free for all staff and students.
